New patient information

We are so excited to have you as a new patient at Novant Health Forsyth Pediatrics! We can't wait to welcome you to our office and deliver the remarkable care your children deserve.

To make your first appointment with a provider in our office, contact the location most convenient for you.

A parent or guardian should accompany your child to her visit. Please bring with you to your child's first appointment:

• Your photo ID.
• The insurance card covering your child.
• Your method of copayment or payment in full if you are uninsured.
• Any prescription or over-the-counter medications your child is currently taking.

ADHD diagnosis
