School, sports & camp physicals

Before your child attends school or participates in a sport or camp, a physical exam might be required to make sure you child will not endanger himself or others while participating in the activity. Novant Health Forsyth Pediatrics regularly completes school, sports and camp physicals and associated forms.

If you have received forms from your child's school, camp or sports team, please bring them with you to your child's appointment, as we will not have access to these forms in our office. Often, these forms can be completed at the same time as your child's annual exam. This can save you time and money. If you need school, camp or sports physical forms completed at another time, there might be an additional charge for having those forms completed.
