Diagnosing ADHD starts with a visit to Novant Health Forsyth Pediatrics to rule out other problems that could be causing ADHD-like symptoms. Then, parents and teachers are surveyed to determine how the behavior is presenting itself. After that information has been assessed, your child's provider will work with your family to create a treatment plan specific to your child. 

If you are visiting one of our offices for a potential ADHD diagnosis or help with an ADHD treatment plan, you will need to complete/have completed by your child's teacher:
